Channels out of phase

What can make the channels get out of phase when installing a new cartridge?
Recently I switched cartridges and with the last one installed I get channels out of phase.
It's very strange since all the connections are correct.:confused::confused::confused:
Any clues?

It is very possible that the manufacturer wire the cartridge out of phase. You can switch the wires on the cartridge leads to get the correct phase.

That's not the case since I've had it installed before and didn't noticed any out of phase.
